Primary Health Care Project

The project builds partnerships with local GPs, pharmacies and primary health care staff to help identify and support carers. It aims to:

Identify carers
Provide carers with appropriate support, guidance and information
Help carers to access other organisations
Provide primary health care staff with training/awareness raising
Build and maintain partnerships with local pharmacies, GP surgeries, medical practices and hospital outpatient clinics
