Primary Health Care Project

The project builds partnerships with local GPs, pharmacies and primary health care staff to help identify and support carers. It aims to:

• Identify carers
• Provide carers with appropriate support, guidance and information
• Help carers to access other organisations
• Provide primary health care staff with training/awareness raising
• Build and maintain partnerships with local pharmacies, GP surgeries, medical practices and hospital outpatient clinics
